Video Questionnaire

This form helps me to understand your work and ensure a good fit in creating a video that genuinely represents you. I review each submission personally, and thoughtful, complete answers give me a good sense of who you are and what approach will work best.

Please read the first question carefully — it clarifies what is necessary for us to proceed.

All information shared with me is kept CONFIDENTIAL.



    To participate, are you willing to:
    1) Obtain a tripod if you don't already have one? (I'll share recommendations if needed.)
    2) Be open to gentle guidance during the session?
    3) Be open to me using the final video on my portfolio? (I will check with you before doing so.)
    4) Share feedback on the process afterward?
    5) Commit to a 2-part process:
    - (1) A Preparation Session (approx. 45 min), and
    - (2) A Recording Session, (about 60 min), done on a separate date?
